The 2 biggest bugs in franchise mode are the draft revert glitch and the 31/32 user bug. When hit with these issues, it can completely ruin your franchise. For some reason on crucial downs the defensive coordinator becomes the QUARTERBACK and the offensive coordinator is stuck on a WR and cannot move or do anything for that play. Basically you need to FIRE your OC the week of the SB... then go to FILL spots on menu to [re]hire the OC (before advancing). Then you can hire (or re-hire) the OC, and you will not get stuck with the FATAL ERROR (you need to repeat ever year).
